2. 건강력

1) 현병력 : 2004년 10월 18일경 hematemesis 있어 local clinic 내원하여 EGD 시행하여 gastric ulcer bleeding care(TACE) 후 퇴원한 환자로 3~4일 전부터 epigastric soreness 있던 중 12월 16일 저녁 4시, 7시경 한 사발가량의 hematemesis 발생하였으며, melena dizziness 있어 local clinic 경유 본원 ER refer 되어 집중관리를 받기 위하여 내원함.

(1) C.C : hematemesis, melena

(2) 의식상태 : Alert, pupil reflex normal

(3) 배설 · 배병 양상 : 배설-foley catheter insert하심. bloody color, moderate amount

배변-침상배설, melena

(4) 피부양상 : abdomen distension보이며 jaundice 양상 나타남.

2) 과거력

DM(diabetes mellituse)-5년 전 진단을 받고 치료중임.
3. 증상 · 증후

① hematemesis ② melena

③ abdomen distension ④ chest discomfort complaint

⑤ 11/16일 11 : 00경 Headache, 전신피로, BT-37.5°C checked

⑥ BST 400~500mg/dl

⑦ hematochezia 봄.

⑧ urine color bloody 함.

⑨ out put poor

⑩ jaundice
